The devolution of health in Kenya has heralded significant political noise around facilities management and HRH unrests across the republic. In response, senior clinical professionals — the most experienced care providers in any health system — are retreating from the boardroom.

What fills that vacuum? Strategy, operations, and tactics left to non-clinicians, junior medics, or senior generalists who have lost touch with frontline patient care. These may not command peer respect among specialists, and they cannot understand the specialist's frustrations at a visceral level.

As a result, the consultant's voice arrives at the boardroom table after three or more rounds of editing — filtered, packaged in corporate parlance — and may miss its essential weight.

Strategic planning and organisational design consultancies operate on this same filtered information. They gather data from clinicians who are barely available — because they are in clinics, theatres, and ward rounds — and validate findings against other hospital plans that went through the same flawed design process.

The Grounded Impact anchor is more than a visual device. It is a statement of intent. In maritime tradition, an anchor is the instrument of stability — it holds a vessel in place against the current, the wind, and the tide.

The Story

From goat-herder to surgeon

Dr. Aruyaru started school at age 9 in rural Meru County. When the time came to join Moi University to study medicine, his family could not afford the fees — and he was enrolled instead in a mechanical course at a polytechnic. His teachers refused to accept that outcome. They protested, fundraised, and ensured he took his place in the medical school. He became the first doctor St Cyprian High School ever produced.

That origin story is not background. It is the engine of everything Grounded Impact does. It is proof that the resources to solve Africa's health challenges are already here — they simply need to be held in the right environment to flourish.

Today, Dr. Aruyaru serves as Chief Medical Officer at PCEA Kikuyu Hospital, overseeing four clinical units and more than 500 staff. As Secretary General of the Surgical Society of Kenya, he chairs the Scientific and Research Committee and drives the organisation's research capacity-building agenda across East and Central Africa.

He is a regular contributor to the Daily Nation, The Standard, and Business Daily, and the author of The Chronicles of a Village Surgeon (2021). His LinkedIn platform commands more than 32,000 followers — one of the largest for a practising clinician in Kenya.
